The Transportation Department at K’ima:w Medical Center (KMC)
is here to provide transportation services to individuals who utilize
the services of KMC, which include
Medical,
Dental and
Outreach
services. Transportation services are provided to assist individuals/families
who are not able to provide their own transportation. It is the goal
of the transportation department to provide the community with the best
possible services. We provide transportation services for appointments
in the Hoopa Valley, Humboldt County and outer metropolitan areas.
VALLEY TRANSPORTATION
Please notify the transportation department that you have an appointment
and will require transportation to the clinic as soon as possible. If
possible try to schedule appointments after 8:30am. For last minute
clinic appointments and trips to Urgent Care we will pick you up ASAP.
Leave a message if nobody is in the office. If we are not in the office,
we are either delivering medication or transporting other clinic patients.
TRANSPORTATION TO EUREKA
The transportation department requires a 24-hour advance notice if you
will require transportation to Eureka. This advance notice allows the
department to make appropriate arrangements for your trip. REFERRAL
REQUIRED.
OUT OF THE AREA TRANSPORTATION
The transportation department should be notified at least 72 hours in
advance if you will be requiring transportation out of the area so that
arrangements can be made. Please speak with the staff member who will
be transporting you out of the area ahead of time so that any specific
arrangements can be made. The transportation department will assist
you in connection with the appropriate transporter. REFERRAL REQUIRED.
CANCELLATIONS
If you will not be going to your appointment or have made other arrangements
for transportation, please notify the transportation department as soon
as possible.
LOCATION WHERE YOU NEED TO BE PICKED UP
If you need to be picked up at a location other than your regular residence,
please notify the department so that we can pick you up in a timely
manner.
FAMILY MEMBERS
Family members may accompany frail elderly patients, physically debilitated
patients, and children/infants on transports.
MEDICATION DELIVERIES
This service is primarily for individuals who have no transportation
or family assistance.
NO PERSONAL STOPS
This includes bank, post office and stores.
